Physician Services in the Home:
Is an appointment or insurance necessary?
You do not have to make an appointment to see a physician , walk-ins are welcome. We accept all healthcare plans, but insurance is not necessary. We accept cash, checks, and credit cards. |
Evaluation and management by physicians practicing medicine at the same level as is offered at a free standing clinic, including:
- Comprehensive evaluations.
- Routine visits.
- Referral to specialists.
- Prescription renewals.
- 24-hour telephone access to physicians.
Lab and Diagnostic Testing:
Most laboratory and diagnostic testing necessary for initial and ongoing management of patients with multi-system chronic disease. This includes in-home:
Blood analysis.
- X-rays.
- Electrocardiography.
- Echocardiography.
- Cardiac monitoring.
- Pulmonary function testing.
- Diagnostic ultrasound and vascular studies.
Home care consultations to office-based physicians:
The patient's primary care physician refers the patient to our medical group for home-care management. As long as the patient remains homebound, we assume responsibility for all medical care except hospitalization.
If the patient requires hospitalization, our referral-tracking system sends the patient to their primary care physician's hospital.
If a referring physician does not wish to follow the patient in the hospital, we have affiliations with physicians available at multiple hospitals in the tri-county area.
Ongoing supervision & Case Management:
Supervision of multi-disciplinary home care services from providers such as nursing, social work, physical therapy, occupational therapy, speech therapy, dietary counseling, home health aides, and durable medical equipment.
Our care coordinating division links multiple sites of service across the continuum of care (emergency room, hospital, skilled nursing facility, and home health agency).
